The forms I’ve been using in my student interview and evaluations for probably 10+ years now were in desperate need of some updating! I wanted to include my new studio logo, and I also wanted to change some of the questions and add a few new ones, notably, finding out from parents what method of communication they prefer and if their child has any allergies (to assist me in providing appropriate snacks at studio events!). I also wanted to find out if students have devices that they can use during the week in case there are specific apps I want to suggest, and also whether they are ios or Android devices. A few of the evaluation questions have also been tweaked based on what has proven more effective when I’m conducting the interviews.
All of the Interview and Evaluation forms are now available for free in the Music Matters Blog store. They are Microsoft Word documents that are fully editable with a place for your own studio logo and location information. Feel free to download, edit, and use in your studio!
