One of the most helpful decisions I made years ago was to schedule Year-End Evaluations for the final piano lesson of the spring semester. One or both parents are required to attend with the student and we use their lesson time to discuss progress over the past year along with plans and goals for the following year. At the beginning of May I send home a questionnaire for the student and parent, and I complete an evaluation for each student.
When we meet at the end of May, these serve to guide our discussion and help address any issues that may need to be resolved. The Student Evaluation forms remain the same each year, but I tweak the questionnaires as needed.
