Student Evaluation Form

Here’s a generic version of a Student Evaluation Form I developed to use at my Year-End Evaluations each spring. I also have each student and parent fill out a separate questionnaire asking for their input on various questions. I made this form more generic so that it can be used at any time of year.

I use the space under each area to write additional comments about what I think the student has done well or what needs improvement. I keep a copy in my file for the student and give the parent a copy to keep for their own records. You can click on the picture of the form to download your own copy for use in your studio.

    Very nice form, Natalie! I’m definitely going to share this with my pedagogy class. It’s a nice way to keep parents and/or students informed on which areas that they need to work on. Sometimes just seeing it in black and white makes a big difference.

    Thanks, Mario! Glad you can use it! It’s really helpful for me, too, to take time to evaluate the students and think through how they are doing in the different areas.

    I have been a piano/voice teacher for 16 years. I am pregnant with my fourth child, and will have to give up teaching for a few years to attend to my family. I am now faced with the prospect of finding a new teacher to “adopt” my beloved students, and have been searching for a detailed evaluation form to send with each student to help their new teacher know exactly what they have been taught, and what they still need to know. I am thinking I may have to make up my own form, but your form gives me a start. I want a more detailed form, (more like a report card) encompassing the areas of muscianship very specifically according to levels. Do you know of anything like this already out there in the pedagogical world?
